Non steroidale analgetika

Uganuća, istegnuća i druge povrede mekih tkiva su česte i za njihovo liječenje su potrebni lijekovi protiv bolova (analgetici). Ti lijekovi najčešće se daju u obliku tablete koja se uzima na usta (oralno). Postoje mnogi oblici oralnih lijekova protiv bolova za liječenje tih povreda, ali nije poznato je li koji bolji u odnosu na druge. U ovom Cochrane sustavnom pregledu ispitan je učinak lijekova protiv bolova na smanjenje boli, oticanja i funkcije kod povreda primjenom nesteroidnih protuupalnih lijekova (engl. non-steroidal anti-inflammatory drugs, NSAID) u usporedbi s paracetamolom, opijatima (primjerice kodein), te s postupcima alternativne medicine ili nekom od kombinacija navedenog. Također su istražene nuspojave ovih oblika liječenja.

The present study examines the influence of the non-steroidal anti-inflammatory drugs (NSAIDs) meloxicam, tolfenamic acid, ketoprofen and the opioid buprenorphine on wound healing in rats compared to a control group treated with saline ( % NaCI) solution. The trial includes 245 male Wistar rats. Euthanasia is performed on day 1, 4, 7, 10, 14, 21 and 28 post operationem. This schedule is chosen to obtain a general view of the development of wound healingTwo polyurethan-sponges are implanted subcutaneously in the left and right dorsolumbar region. The analgesics respective saline solution are given 1 hour pre operationem. Following dosages are used: meloxicam (Metacam, for injection 0,5 %) 0,2 mg/kg/d ., tolfenamic acid (Tolfedine, for injection 4 %) 4,0 mg/kg/d ., ketoprofen (Romefen, for injection 1 %) 2,0 mg/kg/d ., buprenorphine (Temgesic, for injection 0,03 %) 0,3 mg/kg . twice a day and saline solution (for injection 0,9 %) 0,1 ml/animal/day ..The rats" body-weight is assessed daily as well as their general condition and behaviour. The animals treated with tolfenamic acid show a progressive intolerance reaction (swelling at injection area) from day 6 on. The general condition of the buprenorphine group is slightly impaired. Due to sedation both activity and food intake are decreased, which results in a reduction of body-weight (from day 1 to 7).The blood samples taken on the day of euthanasia do not reveal a grave systemic influence or side-effects of the order to describe the development of wound healing a biomechanical and biochemical parameter is measured. The tensile strength of the skin wound is determined by tensiometry and the amount of hydroxyproline in the sponge is established biomechanical results in the present study assume that there is no adverse affect of the NSAIDs meloxicam, tolfenamic acid and ketoprofen on wound healing,whereas the tensile strength of the buprenorphine group is reduced mildly at certain times. Therefore the post surgical use of the actually proved analgesics, which is essential for the well-being of animals and demanded for humane treatment, is absolutely justified.
